Pentru b\u0103uturile cu \u00eendulcitori artificiali, \u00een schimb, rezultatele au fost diferite.<\/strong><\/p>\n\n\n\n<p>Cercet\u0103torii au analizat date din 11 studii prospective, cu peste 1,5 milioane de participan\u021bi, iar rezultatele au fost publicate \u00een revista&nbsp;<a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2850133\" target=\"_blank\" rel=\"noreferrer noopener\">JAMA Network Open<\/a>.<\/p>\n\n\n\n<p>Concluzia principal\u0103 a fost c\u0103 fiecare por\u021bie zilnic\u0103 de b\u0103utur\u0103 \u00eendulcit\u0103 cu zah\u0103r a fost asociat\u0103 cu un risc mai mare pentru anumite forme de cancer hepatic.<\/p>\n\n\n\n<p>B\u0103uturile cu \u00eendulcitori artificiali nu au avut aceea\u0219i asociere, nici \u00een analiza general\u0103 a cancerului hepatic, nici \u00een analiza separat\u0103 pe subtipuri.<\/p>\n\n\n\n<p>Studiul a fost coordonat de Cody Watling, cercet\u0103tor la National Cancer Institute din SUA, \u00eempreun\u0103 cu Katherine McGlynn \u0219i al\u021bi colegi.<\/p>\n\n\n\n<p>Participan\u021bii aveau, \u00een medie, aproape 58 de ani, iar femeile reprezentau pu\u021bin peste jum\u0103tate din e\u0219antion.<\/p>\n\n\n\n<p>Este important de men\u021bionat c\u0103 monitorizarea a fost de lung\u0103 durat\u0103, desf\u0103\u0219ur\u00e2ndu-se aproape 18 ani, perioad\u0103 \u00een care au fost diagnosticate 2.811 cazuri noi de cancer hepatic, dintre care 1.699 de carcinoame hepatocelulare \u0219i 444 de colangiocarcinoame intrahepatice.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">C\u00e2t de mare este, de fapt, riscul<\/h2>\n\n\n\n<p>Pentru fiecare b\u0103utur\u0103 \u00eendulcit\u0103 cu zah\u0103r consumat\u0103 zilnic, riscul de carcinom hepatocelular a fost cu aproximativ 10% mai mare, iar cel de colangiocarcinom intrahepatic, o form\u0103 mai rar\u0103 de cancer \u2013 cu aproximativ 15% mai mare.<\/p>\n\n\n\n<p>Autorii se refer\u0103 la o por\u021bie obi\u0219nuit\u0103 de suc sau alt\u0103 b\u0103utur\u0103 cu zah\u0103r, apropiat\u0103 \u00een practic\u0103 de o doz\u0103 sau o sticl\u0103 mic\u0103, adic\u0103 aproximativ 330-350 ml. Cantitatea nu a fost \u00eens\u0103 identic\u0103 \u00een toate studiile incluse, deoarece analiza a reunit cohorte cu chestionare alimentare diferite.<\/p>\n\n\n\n<p>Procentele descriu \u00eens\u0103 o cre\u0219tere relativ\u0103 a riscului. Nu \u00eenseamn\u0103 c\u0103 o persoan\u0103 care bea zilnic suc va dezvolta cancer hepatic, ci c\u0103, \u00een grupuri mari de popula\u021bie, consumul regulat de b\u0103uturi cu zah\u0103r a fost legat de un risc mai mare pentru aceste dou\u0103 forme de cancer.<\/p>\n\n\n\n<p>Pentru colangiocarcinomul intrahepatic, rezultatul a fost mai aproape de limita semnificativ\u0103 statistic. Consumul zilnic a fost legat de un risc mai mare pentru carcinom hepatocelular, forma cea mai frecvent\u0103 de cancer hepatic primar, care porne\u0219te din celulele ficatului, \u0219i pentru colangiocarcinom intrahepatic, o form\u0103 mai rar\u0103, dezvoltat\u0103 \u00een canalele biliare din interiorul ficatului.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Leg\u0103tura dintre aspartam \u0219i cancerul hepatic<\/h2>\n\n\n\n<p>Pentru b\u0103uturile cu \u00eendulcitori artificiali, precum cola f\u0103r\u0103 zah\u0103r, rezultatele au fost diferite. Cercet\u0103torii au comparat persoanele care consumau cele mai mari cantit\u0103\u021bi cu cele care beau rar sau deloc astfel de produse.<\/p>\n\n\n\n<p>A ap\u0103rut o cre\u0219tere mic\u0103 a riscului, dar aceasta nu a atins pragul de semnifica\u021bie statistic\u0103, nici pentru cancerul hepatic analizat \u00een ansamblu, nici pentru subtipurile evaluate separat.<\/p>\n\n\n\n<p>Datele nu sus\u021bin, a\u0219adar, aceea\u0219i rela\u021bie observat\u0103 \u00een cazul b\u0103uturilor \u00eendulcite cu zah\u0103r. Watling \u0219i colegii s\u0103i noteaz\u0103 \u00een lucrare c\u0103 dovezile privind o asociere \u00eentre b\u0103uturile cu \u00eendulcitori artificiali \u0219i cancerul hepatic, inclusiv \u00eentre aspartam \u0219i carcinomul hepatocelular, \u201enu sunt sus\u021binute conving\u0103tor de aceste analize\u201d.<\/p>\n\n\n\n<p>Cu toate acestea, agen\u021bia pentru cancer a&nbsp;<a href=\"https:\/\/www.who.int\/news\/item\/14-07-2023-aspartame-hazard-and-risk-assessment-results-released\" target=\"_blank\" rel=\"noreferrer noopener\">Organiza\u021biei Mondiale a S\u0103n\u0103t\u0103\u021bii<\/a>&nbsp;a clasificat aspartamul drept \u201eposibil cancerigen pentru oameni\u201d, \u00een iulie 2023.<\/p>\n\n\n\n<p>\u00cen plus, alte&nbsp;<a href=\"https:\/\/www.neurology.org\/doi\/10.1212\/WNL.0000000000214023\" target=\"_blank\" rel=\"noreferrer noopener\">studii<\/a>&nbsp;observa\u021bionale au asociat consumul de b\u0103uturi cu \u00eendulcitori artificiali cu un declin cognitiv mai rapid, leziuni vasculare \u0219i risc crescut de accident vascular cerebral.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Cum ajunge zah\u0103rul s\u0103 afecteze ficatul<\/h2>\n\n\n\n<p>Leg\u0103tura dintre b\u0103uturile \u00eendulcite cu zah\u0103r \u0219i cele dou\u0103 forme de cancer hepatic s-a p\u0103strat \u0219i dup\u0103 ce cercet\u0103torii au \u021binut cont de diabet \u0219i obezitate. Asta \u00eenseamn\u0103 c\u0103 riscul observat nu pare explicat doar prin aceste dou\u0103 diagnostice, de\u0219i ambele sunt str\u00e2ns legate de s\u0103n\u0103tatea ficatului.<\/p>\n\n\n\n<p>\u201ePrivite \u00eempreun\u0103, aceste date consolideaz\u0103 dovezile privind rela\u021bia dintre b\u0103uturile \u00eendulcite cu zah\u0103r \u0219i cancerul hepatic \u0219i las\u0103 deschis\u0103 posibilitatea ca aceast\u0103 leg\u0103tur\u0103 s\u0103 nu fie explicat\u0103 \u00een \u00eentregime prin tulbur\u0103rile metabolice asociate consumului lor\u201d, au precizat, \u00eentr-un comentariu publicat \u00een&nbsp;<a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2850141\" target=\"_blank\" rel=\"noreferrer noopener\">JAMA Network Open<\/a>, Daniel Clayton-Chubb \u0219i Andrew T. Chan, de la Massachusetts General Hospital \u0219i Harvard Medical School.<\/p>\n\n\n\n<p>Explica\u021biile biologice pornesc de la felul \u00een care ficatul proceseaz\u0103 zah\u0103rul lichid, mai ales fructoza. Spre deosebire de glucoz\u0103, care este folosit\u0103 de mai multe \u021besuturi, o mare parte din fructoz\u0103 ajunge direct la ficat. C\u00e2nd aportul este mare \u0219i repetat, ficatul o poate transforma \u00een gr\u0103sime. \u00cen timp, acest proces favorizeaz\u0103 acumularea de gr\u0103sime hepatic\u0103, rezisten\u021ba la insulin\u0103 \u0219i inflama\u021bia.<\/p>\n\n\n\n<p>Clayton-Chubb \u0219i Chan men\u021bioneaz\u0103 mai multe mecanisme posibile: produc\u021bia crescut\u0103 de gr\u0103sime \u00een ficat, modificarea adipokinelor (substan\u021be produse \u0219i eliberate de \u021besutul adipos), schimb\u0103ri ale microbiomului intestinal, cre\u0219terea nivelului de gr\u0103sime visceral\u0103, rezisten\u021ba la insulin\u0103 \u0219i \u00eenc\u0103rcarea cu gr\u0103sime a ficatului. Aceste procese pot ac\u021biona separat sau \u00eempreun\u0103 \u0219i pot crea un mediu biologic favorabil apari\u021biei cancerului hepatic.<\/p>\n\n\n\n<p>Pe scurt, fructoza din b\u0103uturile dulci este transformat\u0103 de ficat \u00een gr\u0103sime. Surplusul se depune, favorizeaz\u0103 rezisten\u021ba la insulin\u0103 \u0219i preg\u0103te\u0219te terenul pentru steatoza hepatic\u0103, numit\u0103 popular \u201eficat gras\u201c, o afec\u021biune care, \u00een timp, poate evolua spre forme grave de boal\u0103 hepatic\u0103.<\/p>\n\n\n\n<p>Spre deosebire de fructele \u00eentregi, unde zah\u0103rul vine la pachet cu fibre care \u00eencetinesc absorb\u021bia, b\u0103uturile \u00eendulcite livreaz\u0103 cantitatea de zah\u0103r rapid \u0219i concentrat.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Leg\u0103tura cu alte tipuri de cancer<\/h2>\n\n\n\n<p>\u00cen 2023, un alt studiu prospectiv publicat \u00een&nbsp;<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2807987\" target=\"_blank\" rel=\"noreferrer noopener\">JAMA<\/a>, realizat pe aproape 100.000 de femei aflate la menopauz\u0103, a raportat mai multe cazuri de cancer hepatic \u0219i mai multe decese prin boli cronice de ficat \u00een r\u00e2ndul participantelor care consumau cel pu\u021bin o b\u0103utur\u0103 \u00eendulcit\u0103 cu zah\u0103r pe zi. B\u0103uturile cu \u00eendulcitori artificiali nu au avut aceea\u0219i rela\u021bie cu riscul. Rezultatul era, a\u0219adar, apropiat de cel observat \u00een analiza publicat\u0103 acum \u00een&nbsp;JAMA Network Open.<\/p>\n\n\n\n<p>B\u0103uturile cu zah\u0103r au fost analizate \u0219i \u00een leg\u0103tur\u0103 cu alte tipuri de cancer. Unele studii observa\u021bionale le-au asociat cu un risc mai mare de cancer oral \u0219i cu un risc crescut de cancer colorectal cu debut timpuriu la femei.<\/p>\n\n\n\n<p>Aceste rezultate nu demonstreaz\u0103, singure, o rela\u021bie cauzal\u0103, dar se adaug\u0103 unui num\u0103r tot mai mare de date care leag\u0103 consumul frecvent de b\u0103uturi dulci cu bolile metabolice \u0219i hepatice.<\/p>\n\n\n\n<p>Fiind vorba despre studii observa\u021bionale, \u00een cazul cercet\u0103rii privind leg\u0103tura dintre b\u0103uturile cu zah\u0103r \u0219i cancerul hepatic nu se poate dovedi o rela\u021bie de cauzalitate, ci doar o asociere.<\/p>\n\n\n\n<p>\u00cen plus, \u00een majoritatea grupurilor, consumul de b\u0103uturi a fost m\u0103surat o singur\u0103 dat\u0103, la \u00eenceput, de\u0219i obiceiurile alimentare se schimb\u0103 de-a lungul anilor. La fel, diabetul a fost consemnat tot la un singur moment.<\/p>\n\n\n\n<p>Cercet\u0103torii recunosc \u0219i c\u0103 nu au putut \u021bine cont de bolile hepatice preexistente, precum ficatul gras de cauz\u0103 metabolic\u0103 sau ciroza.<\/p>\n\n\n\n<p>\u201ePrin urmare, ca \u00een cazul tuturor studiilor observa\u021bionale, aceste rezultate pot fi influen\u021bate de factori de confuzie reziduali \u0219i nem\u0103sura\u021bi\u201d, au subliniat autorii.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Cancerul hepatic, \u00een topul neoplasmelor cu cel mai mare num\u0103r de decese<\/h2>\n\n\n\n<p>\u00cen Rom\u00e2nia, consumul mediu de b\u0103uturi r\u0103coritoare a sc\u0103zut, \u00een 2024, cu 3,9 litri pe cap de locuitor fa\u021b\u0103 de anul anterior, adic\u0103 cu 3,3%, potrivit&nbsp;<a href=\"https:\/\/insse.ro\/cms\/sites\/default\/files\/field\/publicatii\/consumul_de_bauturi_in_anul_2024.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Institutului Na\u021bional de Statistic\u0103<\/a>.<\/p>\n\n\n\n<p>Consumul total de b\u0103uturi nealcoolice, categorie care include apele minerale, b\u0103uturile r\u0103coritoare \u0219i berea f\u0103r\u0103 alcool, a fost mai mic cu 9,3 litri per locuitor.<\/p>\n\n\n\n<p>\u00cen acela\u0219i timp, cancerul hepatic r\u0103m\u00e2ne una dintre tumorile cu mortalitate mare \u00een \u021bara noastr\u0103. De altfel, \u00een Rom\u00e2nia se \u00eenregistreaz\u0103 cea mai mare rat\u0103 de inciden\u021b\u0103 din UE \u00een ceea ce prive\u0219te cancerul hepatic \u00een r\u00e2ndul femeilor, potrivit&nbsp;<a href=\"https:\/\/www.oecd.org\/content\/dam\/oecd\/ro\/publications\/reports\/2025\/02\/eu-country-cancer-profile-romania-2025_ef833241\/a90c2c0d-ro.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Profilului de \u021bar\u0103 privind cancerul 2025<\/a>.<\/p>\n\n\n\n<p>Estim\u0103rile&nbsp;<a href=\"chrome-extension:\/\/efaidnbmnnnibpcajpcglclefindmkaj\/https:\/gco.iarc.who.int\/media\/globocan\/factsheets\/populations\/642-romania-fact-sheet.pdf\">GLOBOCAN 2022 pentru Rom\u00e2nia<\/a>, publicate de International Agency for Research on Cancer (IARC), indic\u0103 3.682 de cazuri noi de cancer hepatic \u00eentr-un an, adic\u0103 3,5% din totalul cancerelor, \u0219i 3.495 de decese, echivalentul a 6,2% din mortalitatea oncologic\u0103.
